How alternative tobacco and nicotine products work, and how their risk profile differs from cigarettes

Heated tobacco, nicotine pouches and vapes: what actually happens inside each device, and what WHO and the FDA say about it.

These products share one thing: no combustion. But they are built differently, and that shapes their risk profile.

Heated tobacco systems heat specially made tobacco sticks instead of burning them. There is no smoke, but there is a nicotine aerosol that the user inhales. The temperature is lower than in a cigarette, and the aerosol carries lower levels of harmful chemicals than cigarette smoke (WHO, 2020).

Nicotine pouches are small pouches of nicotine powder with no tobacco leaf. You place one between the gum and the lip, and nicotine is absorbed through the lining of the mouth. Nothing is burned or inhaled.

Vaping products heat a liquid with nicotine and flavourings, but no tobacco, turning it into an aerosol.

Here it matters not to oversimplify. WHO states that lower toxicant levels do not necessarily mean lower health risk, and that long-term effects are unknown. In 2020 the US FDA allowed one heated tobacco system to be marketed with a reduced-exposure claim — a claim about exposure, not about disease risk. None of these products is risk-free. Stopping completely remains the lowest-risk option. E-cigarette sales are prohibited in Kazakhstan.

A useful question for tonight. What do I actually know about the product I choose — and where did that information come from?

When to talk to a doctor. If you want to lower your health risk, discuss a quitting plan with a doctor: there are evidence-based approaches that improve the odds. For urgent symptoms call 103 or 112.
